Assisting Members by phone, in person, and over the computer, you can thrive personally and professionally as you find your true calling in our Health Partner role. To realize success in this role, you must have one or more of the following skill sets:
Taking into account your interests, experience, and skills, our hiring manager will provide you with the resources you need to most effectively coach, educate, and care for the needs of our members. Your responsibilities may include assuring proper member use of our network and coordination of its resources, referring members to appropriate community resources, development and execution of plans of care, and helping members define and develop health goals.
LCSW, LMSW, or LPC
As a part of our Integrated Care Team, you will personally assist members and their families dealing with a wide variety of issues to meet them wherever they are on the journey to health. Serving as a liaison to medical homes, physician practices, and community partners, you will help members referred to us after hospital stays and ER visits who are dealing with psychosocial issues and/or have chronic conditions that put them at high risk for substance abuse and mental-health problems.
