Meet Your Health Partners: Mary Ullrich

Longtime nurse finds new way to provide care as a MissionPoint Health Partner

After 30 years as a nurse, Mary Ullrich was ready for a change but not a loss of mission – she still wanted to provide care and help people get well. Becoming a Health Partner on MissionPoint’s Ambulatory team was just what the she was looking for.

“I was a post-partum nurse for many years, and then moved into lactation consulting for the last decade,” Mary says. “But I wanted to move away from the active-floor nursing work. At the same time, I wanted to keep working with people, because like all nurses I love having that connection. Now I still have that, but instead of working with newborn babies and their mothers, I work with adults who need the same encouragement and support as they face challenges.”

Mary enters a MissionPoint member’s life after he or she has had an emergency department visit or a hospitalization. She learns the member’s medical history, and then checks in to see if all discharge instructions are being followed. If not, she helps cover any gaps, such as prescriptions not filled or doctor’s appointments not kept. When needed, she brings in Health Partners from the Transition and Integrated Care teams to help.

“There are so many ways we can be of service to that member, because all the teams do so much,” she says. “I’m there during that acute transition period, when they are just getting home and may still be a little foggy on all that they need to do. My goal is to make sure that, if they went to the emergency department, they don’t go back again. And if they were hospitalized, I want to make sure they are doing everything they should be during those important first few days back at home. So many times people can fall through the cracks, and we work very hard to make sure that doesn’t happen.”

While she loves seeing members get back to their normal lives, Mary says she is also  happy when she can identify a problem that falls outside her scope of duties and get another Health Partner involved.

“It really is wraparound care,” she explains. “When they are having trouble with their insurance or prescription coverage, we can help. If they want to tackle something like diet and exercise because that’s blocking them from their best health, we can bring in one of our Wellcoaches-certified Health Partners to get that going. No matter what’s going on, there’s a Health Partner who can step in and take over, or get that member pointed in the right direction. It’s what we’re here for, and I love being a part of that.”
