Health Partner helps Members overcome obstacles and achieve wellness
For two years, Chris Mims has worked hard to help MissionPoint Health Partners Members achieve their personal healthy living goals. Because of his perseverance, he’s become known as the “get it done” guy to the physicians and Members he works with on a daily basis.
Many MissionPoint Health Partners work out of physicians’ offices so they can meet Members while they are there for appointments. Whether it’s an in-person meeting, or speaking with someone by phone, Chris says he likes to get right down to what’s going on — and find a solution.
“I wanted to do more care management, and MissionPoint was a perfect fit for me,” says Chris, a registered nurse. “I get to work with Members in several ways: Some I meet at the doctor’s office, and others I contact 30 days after they’ve been sent home from a hospital stay, or have just gotten through an emergency room visit.”
Chris works with physicians in a three-floor practice, as well as an on-site medical clinic, so he spends a lot of time moving around. Even so, he makes sure that he’s reachable by phone so that if a Member needs him he can pitch in right away.
“Sometimes a Member has trouble getting through to the doctor by phone, or they run into an issue at home and are confused,” he says. “I can get right to the nurse and help them with whatever they need. Sometimes that’s a prescription question, other times it’s a home-health issue. They call me the ‘get it done guy’ because I’m not going to stop until we’ve got the issue resolved. I’m definitely getting my wish in terms of hands-on case management!”
Time is of the essence
Chris knows that when someone isn’t feeling well, or has a medical question, they want an answer quickly. That’s why he works hard to be an interface between a busy doctor’s office and MissionPoint Health Partners Members.
“A Member may need to determine if they should be taking a medicine, or if their symptoms mean they should come into the doctor’s office,” Chris explains. “Those are questions that I can answer, or that I can have answered by one of the nurse practitioners. Then we might get that Member to come in, or send them to an urgent-care clinic near their house, so the issue is resolved as quickly as possible.”
He also gets out to Members’ homes, allowing him to see the whole picture of someone’s health. That often means engaging other Health Partners for social services or other needs.
“I might just do a fall-risk assessment, or if there are other issues we can step up how we’re helping that Member,” Chris says. “When I see someone in the doctor’s office, I might not know that they’re not taking their medicines properly, or that there’s a problem in their home that puts them at risk. When I go there, I can really see what’s going on.”
Whatever the case, every day is different and that’s what drives Chris to succeed.
“I get to see people here and in their homes, and also talk to them by phone, so I touch their lives in lots of ways,” he says. “I learn about them; what their goals are, and how they want to feel better. Then I can work with them on the steps we need to take to make that happen.”
